Student-Athlete Sports Documentation

All SCISA schools are mandated to have all athletic records available online, allowing easier access to documents and reducing the risk of misplacing forms.

When parents complete these online forms, they will be automatically sent to the CBS Athletic Department. Parents, you will need only to print out the Physical Examination Form and History and take it to your physician or bring it to our physical day at the school.

Please click the links below and fill out ALL of the necessary forms. The forms and fee must be received before your child starts the first practice day of their sport. For volleyball, we must receive forms before school starts on August 9 and your Athletic Fee of $150. They cannot attend practice and participate in any of the below sports unless these are done. This fee can be given to Lisa Ingle in the Finance Office or brought to the School Office marked: Athletic Fee.
